Home Health

What is Home Health?

Your environment can have a big impact on your recovery from illness or injury. Home healthcare helps you manage your care in your home under the direction of a skilled healthcare provider. That way you’re able to recover in a familiar environment where you feel comfortable. The end goal of home healthcare is to help you regain your independence without having to move to a hospital or other inpatient care.

Your course of treatment will be customized to your needs and goals and may include treatments like:

  • Skilled nursing
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Medical social services
  • Home health aides
  • Disease state management

What to Expect

We pride ourselves in going above and beyond to treat our patients with compassion and dignity.

Work with your doctor

Care always starts with a referral from a healthcare provider. A prescription will allow us to start your care and help us work with your insurance.

The first visit

On your first visit we’ll take a comprehensive assessment of your home and your physical health. From there we’ll talk about goals and develop a plan of care that day.

Work together

Once we have our plan we’ll begin working with you regularly to help you reach your goals.

Discharge

Once you’ve accomplished your goals, we’ll work together to ensure you have everything you need to continue your care and independence after you’re discharged.

Frequently Asked Questions

Your doctor will prescribe home healthcare. From there, you can choose to work with us. Just fill out a contact form, call us at 1-800-845-5850, or find a nearby service provider. Once we're in touch we'll handle coordinating all your care and working with your doctor.

Home healthcare is generally covered at 100% under Medicare/Medicaid. If you have a different type of insurance, contact us and we'll be able to give you more details about your coverage.

Our home healthcare consists of a customized plan. We'll come to your home for an initial visit to learn more about your needs and goals. From there we'll develop a plan of care based on what we learned and we'll put that plan into place the same day.

After you've reached your health goals, we'll also be able to provide disease management plans, follow-ups, and ensure you have what you need to continue living healthily and independently in your home.

Home healthcare is designed for patients who are currently homebound or are having difficulties leaving their home to take care of their basic needs. Your doctor will need to prescribe home healthcare.

We provide skilled nursing and therapy services (Physical Therapy, Occupational Therapy and Speech Therapy) services to patients who are considered “homebound” by their physician. If you are unsure if you meet the necessary qualifications, we can have a nurse or therapist complete an evaluation to determine if you qualify for Home Healthcare services.

If homebound status is questionable, our registered nurses can provide an in-home assessment to determine your status:

Do you require skilled nursing and/or need special assistance with activities of daily living?
Are you unable to leave home unassisted?
Do you require part-time, intermittent or daily care?
Are you under the care of a physician?
Has your physician written an order requesting these services?

We're Not Just Another Healthcare Provider

We treat each patient as an individual and work hard to help them meet their healthcare goals.

Quick Response

When your healthcare provider refers you to us we contact you as soon as possible to coordinate care.

We Manage Everything

Once you begin working with us, we're able to seamlessly coordinate any other services you may need in your home.

Superior Care

Our clinicians genuinely care about their patients. We go above and beyond for you and your loved ones.

Community Oriented

We have roots in your town. We live in and provide care in your community.

Did you know?

Patients who receive home care are less likely to be readmitted to a hospital than patients who don't.
Your physical therapist was very knowledgeable about what needed to be done in the exercises he gave me to help me get stronger. Any questions I had along the way he listened and always tried to find a solution to my problem. Thanks to him, now I can walk with confidence and even walked a short distance without a cane!!!
