What is Home Health?

Nursing & Therapy Services Provided in the Home Setting

Home health care allows you to manage your care in your own home under the direction of a skilled healthcare provider.

Life can take you along an unknown and unpredictable path. Your environment can have a big impact on your recovery from illness or injury. We offer a variety of home health care services to help you recover from an injury, rehab from surgery, or help you manage a chronic condition/illness.

If you meet the eligibility requirements, in-home health services are typically covered by Medicare, Medicaid and many private insurance plans.

Home health services can be provided in the comfort of your own home – whether that’s a house, an apartment, a relative’s home or a senior living community. That way you’re able to recover in a familiar environment where you feel most comfortable. The end goal of home healthcare is to help you regain your independence without having to move to a hospital or other inpatient care.

Your in-home treatment will be tailored to your individualized needs, goals and your schedule. Depending on your doctor’s orders, your services could include the following:

  • Skilled Nurse (MSN / LPN / RN / APRN / OCN) - for specialized medical care needs
  • Physical Therapist (PT or PTA) - to help restore mobility and rehab from surgery
  • Occupational Therapist (OT or OTA) - to improve the ability to perform everyday activities
  • Speech Therapist (SLP or SLPA) - to help with speech, language, voice, cognition, and swallowing issues
  • Medical Social Worker (MSW) - to help patients and their families connect with community resources available to them
  • Home Health Aide (HHA) - to help provide personal care assistance with everyday activities (when necessary)

Getting Started

Setting up home health care always starts with a referral from your healthcare provider. A written order or prescription from your doctor will allow us to start your care and help us work with your insurance benefits.

Initial Visit

On your initial visit, we’ll take a comprehensive assessment of your overall health and learn what you need to feel comfortable and safe. From there, we’ll develop a specialized plan of care tailored for your specific needs.

Working Together

Once we have developed your specialized plan of care, we’ll begin working with you regularly to ensure you can remain comfortable and safe. We’ll also be able to adjust your skilled care services as needed.

Patient Discharge

After you’ve accomplished your individualized goals and discharged from our services, we’ll work together to ensure you have everything you need to continue your own care and independence.

Frequently Asked Questions

Your doctor will prescribe home healthcare. From there, you can choose to work with us. Just fill out a contact form, call us at 1-800-845-5850, or find a nearby service provider. Once we're in touch we'll handle coordinating all your care and working with your doctor. 

Home healthcare is generally covered at 100% under Medicare/Medicaid. If you have a different type of insurance, contact us and we'll be able to give you more details about your coverage. 

Our home healthcare consists of a customized plan. We'll come to your home for an initial visit to learn more about your needs and goals. From there we'll develop a plan of care based on what we learned and we'll put that plan into place the same day. 

After you've reached your health goals, we'll also be able to provide disease management plans, follow-ups, and ensure you have what you need to continue living healthily and independently in your home. 

Home healthcare is designed for patients who are currently homebound or are having difficulties leaving their home to take care of their basic needs. Your doctor will need to prescribe home healthcare. 

We provide skilled nursing and therapy services (Physical Therapy, Occupational Therapy and Speech Therapy) services to patients who are considered “homebound” by their physician. If you are unsure if you meet the necessary qualifications, we can have a nurse or therapist complete an evaluation to determine if you qualify for Home Healthcare services.

If homebound status is questionable, our registered nurses can provide an in-home assessment to determine your status:

Do you require skilled nursing and/or need special assistance with activities of daily living?
Are you unable to leave home unassisted?
Do you require part-time, intermittent or daily care?
Are you under the care of a physician?
Has your physician written an order requesting these services?
